Repairing the large and retracted rotator cuff tear

Abstract

Rotator cuff tear is a common problem leading to pain and weakness in shoulder in elderly patient. Patient suffering from rotator cuff tear can either be asymptomatic initially or having the symptoms resolve after a short course of conservative treatment. Unfortunately, the tear progresses with time. Thus, it is not uncommon to find large and retracted tear at a later stage of life. Arthroscopic assisted intervention can sometimes be challenging. The following presentation describes a “cookbook” approach in addressing this common problem through shoulder arthroscopy with video illustration. The surgical steps include: (1) Bursectomy; (2) Tear assessment and classification; (3) Surgical release; (4) Debridement of degenerated tendon; (5) Preparation of footprint; (6) Planning of surgical repair (anatomical vs non-anatomical); (7) Execution of repair.
