AbstractObjective: We report our technique, indications and results of 4 patients whom we performed high flow bonnet vascular bypasses to replace the internal carotid arteries. Method: We describe our indications, operative technique and results for bonnet bypasses in 4 patients. These patients were (1) Patient with advance nasopharyngeal carcinoma and carotid blow out, (2) Pediatric patient with large head and neck tumor, (3) Two patients with recurrence of nasopharyngeal carcinoma. The need for bonnet bypasses were because of local advance disease making ipsilateral vascular bypass impossible. Result: All 4 patients had uneventful peri-operative courses without any ischemic event. All 4 patients went on to receive second stage operation to remove the tumors and the affected internal carotid arteries. Conclusion: Bonnet bypass is a safe and effective neurosurgical technique in replacing diseased internal carotid arteries in patients with advance head and neck tumor.
