AbstractPapillary tumour of the pineal region (PTPR) is an infrequent lesion and is described as a distinct pathological entity in the 2007 World health Organization classification. We encountered a 45-year-old man who was diagnosed to have pineal region tumour initially presenting with symptoms of hydrocephalus and was managed with ventriculoperitoneal shunt. Observation over 15 years showed slow progression in tumour size on MRIs. Near total tumour excision was performed. Microscopic examination showed a papillary tumour of intermediate differentiation. Cytoplasm was palely eosinophilic to vacuolated. Tumour cells are positive for epithelial membrane antigen (EMA), synaptophysin. The location of the tumor and the pathological findings confirm that the diagnosis was PTPR. © 2012 International Society of Neuropathology.
