Abstract | This paper critically examines a local community-based End-of-Life (EoL) Care Pathway program that manages care transitions between residential care homes and hospitals to enhance care quality and promote dignity at the end of life. Using purposive sampling, four groups of stakeholders including Service Users, RCHE Staffs, Medical Professionals and Management Administrators involved in a local EoL Program for RCHEs were recruited to participate in four focus group interviews (N=30). Data analysis yielded three major themed categories that illustrate the system dynamics needed for successful implementation of community-based EoL care pathway in Hong Kong. Implications for policy and clinical practice will be shared. |
